Request for Mass IntentionList Item 1
This form should be used to request a Mass Intention for a Mass at St. Patrick Church or St. Michael Mission.
This form may be completed and saved as a PDF document, ready to print or mail with your payment to the parish office.

Request for Sacramental certificate
This form should be used to request a copy of a certificate for a previously administered Sacrament -- a baptism, First Communion, Confirmation, or Marriage.
This form should be completed and mailed or delivered to the parish office. Allow 5-10 business days for processing.

Parish Facility Use Agreement
Use this form for any event that you are considering in our Parish Hall. The form is for your information only. You will need to meet with a representative of SPRCP at least six months prior to your event to work out the details and discuss any questions you may have.
